If you find that you have been logged out, you will be required to provide your Username then Password to log back into the Wellsmith app. Your Username can be found in the email you received during registration.
If you have forgotten your password and are unable to log back into the Wellsmith app, select Forgot Password? You will be required to provided the Email Address that is associated with your Wellsmith care plan.
After you provide your Email Address and select Send, you will receive an email that contains your Username and the ability to navigate to a website that will allow you to reset your password.
Once you have reset your password, open the Wellsmith app, and log in with the Username in the email and your newly reset Password.
